Job Benefits Day Shift:
Monday - Friday 8:30a -3:30p Competitive Wages Medical Coverage (90% employee / 80% dependents) Paid Time Off (three weeks + personal days) Holiday Pay (15 days) 403(b) Retirement Plan Match (2%) Short-Term and Long-Term Disability Insurance Paid on-the-job training Job Overview Seeking a program nurse to complete direct care duties at Forward Journey Adult Day Services in New Castle and substitute nurse when needed at our Smyrna site. Includes a variety of nursing duties and patient care to encourage participant development, maintain a safe environment, and interact with families, the Division of Developmental Disabilities Services (DDDS), therapists and others. Essential Duties Provides nursing services to meet the participant's special health needs including, but not limited to, J/G-tube feedings, dispensing/ documentation of necessary medication, suctioning, monitoring of seizures and CPR. Assists with maintenance of appropriate/required participant records in electronic/hard copy files, communicates with parent/guardian in relation to participant needs/progress. Communicates with doctors & family as necessary, secures medications/medical supplies & ensures adequate supplies are maintained. Writes nursing notes. Assesses the health and nutritional needs & monitor wellness of program participants. At direction of supervisor - assists with new participant intake - working with families, case managers, drafting ELP, setting up meeting. Alerts other nurse, senior staff and Program Associates as needed to changes in daily physical and health condition of participants. Follows policies and procedures dictated by the Delaware Board of Nursing and DDDS. Supervises Program Associates in required med passes as per DDDS protocol for Medication Administration Certification. Conduct thorough patient assessments, including vital signs, physical examinations, and medication reconciliation to ensure optimal health management. Administer medications, injections, IV infusions, and catheter care following strict aseptic techniques and safety protocols. Monitor patients' conditions closely, utilizing EMR (Electronic Medical Records) systems such as Epic or Cerner to document medical records accurately and efficiently. Collaborate with team members to develop individualized care plans tailored to each participant's needs, including dementia and memory care strategies. Provide health coaching and education to participants and their families on managing chronic conditions such as diabetes, hypertension, or dementia. Assist with diagnostic evaluations like laboratory specimen collection, radiology referrals, or other medical imaging procedures as needed. Ensure compliance with HIPAA regulations and infection control standards while maintaining a safe environment for all participants. Qualifications One (1) year licensed LPN or RN minimum One (1) year of experience working with individuals with disabilities Pass a 70lb lift test Valid Driver's License Pass 5-panel drug test, PPD (TB) test, criminal background check, Medicaid Fraud check, and Adult/Child Abuse Registry checks Proficient with Microsoft Office products and general office machinery.
