Job Summary TidalHealth Home Care Delaware is seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our home care team. You will manage a dedicated caseload of patients, assess their needs and direct highly personalized, one-on-one care in the comfort of their homes. This is a vital role where clinical excellence meets compassionate care, allowing you to build meaningful relationships while ensuring the highest standards of regulatory compliance and patient outcomes.

Key Responsibilities Comprehensive Assessments:

Conduct initial and ongoing OASIS assessments to evaluate patient health status, home environment, and social support systems.

Individualized Care Plan Development:

Design, execute, and continuously update individualized plans of care in collaboration with patients, families, and attending physicians.

Care Coordination:

Act as the central point of contact, bridging communication between doctors, physical therapists, home health aides, and community resources.

Clinical Excellence:

Perform skilled nursing interventions, including wound care, medication management, IV therapy, and vital sign monitoring.

Compliance & Documentation:

Maintain accurate, timely, and compliant medical records using agency software -

Homecare Homebase Patient Advocacy & Education:

Empower patients and their caregivers through health education, disease management strategies, and preventative care practices.

Qualifications Education:

Graduate of an accredited Registered Nursing program

Licensure:

Active, unencumbered RN license eligible for employment in the

State of Delaware Experience:

Minimum of 2 years of nursing experience, with at least 1 year in home health, hospice, or acute care case management.

Key Skills:

Strong proficiency with OASIS documentation, excellent critical thinking, and the ability to work autonomously.

Requirements:

Pay:

$85,000.00 - $115,000.00 per year
