We are seeking a compassionate, highly skilled, and experienced RN Case Manager to join our elite care team. This role is designed for a dedicated professional who thrives in a one-on-one environment and possesses advanced nursing skills. Job Summary TidalHealth Home Care Delaware is seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our home care team. You will manage a dedicated caseload of patients, assess their needs and direct highly personalized, one-on-one care in the comfort of their homes. This is a vital role where clinical excellence meets compassionate care, allowing you to build meaningful relationships while ensuring the highest standards of regulatory compliance and patient outcomes.
Key Responsibilities Comprehensive Assessments:
Conduct initial and ongoing OASIS assessments to evaluate patient health status, home environment, and social support systems.
Individualized Care Plan Development:
Design, execute, and continuously update individualized plans of care in collaboration with patients, families, and attending physicians.
Care Coordination:
Act as the central point of contact, bridging communication between doctors, physical therapists, home health aides, and community resources.
Clinical Excellence:
Perform skilled nursing interventions, including wound care, medication management, IV therapy, and vital sign monitoring.
Compliance & Documentation:
Maintain accurate, timely, and compliant medical records using agency software -
Homecare Homebase Patient Advocacy & Education:
Empower patients and their caregivers through health education, disease management strategies, and preventative care practices.
Qualifications Education:
Graduate of an accredited Registered Nursing program
Licensure:
Active, unencumbered RN license eligible for employment in the
State of Delaware Experience:
Minimum of 2 years of nursing experience, with at least 1 year in home health, hospice, or acute care case management.
Key Skills:
Strong proficiency with OASIS documentation, excellent critical thinking, and the ability to work autonomously.
Requirements:

Pay:
$85,000.00 - $115,000.00 per year
