Job Description

Big Bend Hospice (BBH) is your "Hometown Hospice," providing compassionate end-of-life care for over 40 years to the Big Bend region. Guided by our values of integrity, respect, and excellence, we are seeking a dedicated and experienced RN Case Manager II to join our Clinical Administration team to cover out new western Service areas in Central Time. The ideal candidate for the RN Case Manager II role is a compassionate and highly skilled clinician who excels at coordinating care across settings while supporting patients and families through the hospice journey. This individual serves as both a clinical expert and care coordinator, ensuring seamless transitions from admission through ongoing care, while promoting quality, compliance, and an exceptional patient experience.
Position Duties and Responsibilities:
The RN Case Manager II's responsibilities will include, but are not limited to the following: Evaluates patients and families for hospice admission using approved criteria and procedures Conducts pre-assessment and admission visits in home, hospital, and facility settings Coordinates with referring physicians, intake teams, and community partners to facilitate timely admissions Provides education to patients, families, and referral sources regarding hospice philosophy, eligibility, and services Ensures all admission documentation is accurate, complete, and submitted within required timelines Develops, coordinates, and implements individualized interdisciplinary group (IDG) plans of care Provides skilled nursing care, symptom management, and patient/family education aligned with hospice best practices Maintains consistent communication with physicians, IDG members, and community partners to ensure continuity of care Supervises and provides guidance to Licensed Practical Nurses (LPNs) and Hospice Aides (HAs), including initiating plans of care Participates in interdisciplinary team meetings, quality initiatives, and continuous improvement efforts Demonstrates competency in pain and symptom management, end-of-life care, and holistic patient support Maintains accurate, timely, and compliant documentation within the electronic medical record (EMR) Ensures compliance with Medicare, Medicaid, and all applicable regulatory requirements Adheres to all BBH, state, and federal policies and procedures Serves as a clinical resource and mentor to peers, new hires, and students Collaborates with Admissions, Education, and Community Relations teams to support smooth care transitions Maintains a balanced caseload aligned with organizational standards Participates in weekend, evening, and holiday rotations as assigned Demonstrates accountability, sound clinical judgment, and effective prioritization Utilizes resources and supplies in a fiscally responsible manner Represents Big Bend Hospice with professionalism and compassion Performs other duties as assigned
Work Schedule:
Monday-Friday:
8:00 am- 5:00 pm.
Travel Requirement :
You must have a reliable vehicle and be willing to be dispatched throughout our service areas . This position will cover the following counties: Gadsden Liberty Calhoun Jackson Gulf Bay Holmes Washington other counties as assigned
Education & Experience:
Minimum of one (1) year of inpatient nursing experience required Hospice, home health, or long-term care experience preferred Active Florida Registered Nurse (RN) license required

English Requirements:
