In this role, you'll have the unique opportunity to provide comfort, dignity, and support to individuals facing advanced illness—while working alongside a collaborative, mission-driven team.
Your schedule would be:
8:30am - 5pm & every other weekend Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness. What you'll Do As an RN Case Manager, you will play a vital role in coordinating care and supporting patients and families throughout their hospice journey.
You will:
Lead with purpose: Uphold and embody Trustbridge Hospice's mission, vision, and values every day Coordinate comprehensive care: Manage and oversee patient and family care under the direction of the physician and interdisciplinary team Assess and anticipate needs: Identify physical, emotional, and psychosocial needs of patients and families Develop personalized care plans: Collaborate with the interdisciplinary group (IDG) to create and implement individualized plans of care Provide hands-on care and education: Utilize strong clinical, teaching, and intervention skills to maximize patient comfort and quality of life Champion compassionate service: Deliver personalized, sincere, and engaging support to every patient and family Advocate for patient satisfaction: Address concerns promptly and ensure the highest level of care experience Maintain trust and confidentiality: Respect the privacy and dignity of every patient and family member What You'll Need Registered Nurse currently licensed in the State of Florida. BSN preferred Hospice certification preferred. Current Basic Life Support Certification. One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred. Understanding of and ability to apply characteristics of
Hospice Nursing:
case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs Basic computer skills and EMR software experience a plus. Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license. Why Join Empath Health?
