Applicants who do not meet these qualifications will not be considered. An exciting opportunity is available for an experienced Registered Nurse to deliver compassionate home health care on a contract basis. This role focuses on providing quality nursing services directly to patients in the comfort of their own homes, ensuring individualized patient support through every step of the care process.
Schedule/Hours:
Contract position with flexible scheduling to accommodate patient and nurse needs.
Qualifications:
Active RN license At least 3 months of recent home health nursing experience utilizing OASIS documentation HCHB (Home Care Home Base) experience is highly desirable Demonstrated clinical skills in IV therapy, wound care, and medication administration Proficiency in electronic medical record (EMR) systems Strong communication and interpersonal abilities Capable of working independently and managing time efficiently Reliable transportation and a valid driver's license are required
Key Responsibilities:
Deliver comprehensive nursing care in patient homes, including assessments, vital signs monitoring, and medication management Conduct initial and follow-up patient assessments, developing and adapting care plans to meet evolving patient needs Provide patient and family education on disease management, treatment plans, and preventive health measures Maintain accurate, timely clinical documentation in EMR systems Communicate promptly with physicians and other healthcare professionals to coordinate care Monitor patient conditions, reporting significant changes to supervising providers Ensure all care delivered complies with state and federal home health regulations
