Davenport and surrounding Polk County areas Matrix Home Care is seeking experienced Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to support skilled nursing visits in the Davenport / Polk County area. Davenport is one of our growing referral areas, and we are looking to build a strong field nursing team to support patient care needs in the area.

Position Summary:

The RN/LPN will provide skilled nursing services to patients in their homes, complete visits as assigned, support care coordination, and ensure accurate and timely clinical documentation. This role is ideal for a nurse who enjoys working independently in the field, providing compassionate care, and supporting patients in a home setting.

Responsibilities:

Provide skilled nursing visits in accordance with physician orders and the patient's plan of care. Complete Start of Care, evaluation, follow-up, and routine visits as applicable by license and scope of practice. Monitor patient condition and report any changes to the clinical team. Assist with medication management, wound care, patient education, and other skilled nursing needs. Complete accurate and timely clinical documentation. Educate patients and families on disease management, medications, safety, and home care needs. Coordinate with the clinical team to support continuity of care. Maintain compliance with home health standards, company policies, and regulatory requirements.

Qualifications:

Active Florida RN or LPN license in good standing. Home health experience preferred. Experience with skilled nursing visits preferred. OASIS / Start of Care experience preferred for RN candidates. Strong clinical assessment and documentation skills. Ability to work independently in the field. Reliable transportation and valid driver's license. Professional, dependable, and patient-focused.

Schedule:

Per diem opportunities available. Flexible scheduling based on patient needs and nurse availability. Coverage needed in Davenport and surrounding Polk County areas.
