In this vital role, you will deliver high-quality, patient-centered care directly in patients' homes, ensuring compliance with Medicare guidelines and promoting optimal health outcomes. This position offers flexibility and the opportunity to make a meaningful difference in the lives of individuals requiring skilled nursing services. As a Per Diem Nurse, you will be responsible for conducting comprehensive assessments, developing personalized care plans, and providing education to patients and their families. Your expertise will support our mission to deliver exceptional home health care that enhances independence and quality of life. Duties Perform thorough patient assessments, including vital signs, physical examinations, and review of medical history using EHR (Electronic Health Records) systems such as Axxess. Develop and implement individualized care plans aligned with Medicare standards, ensuring accurate documentation of patient progress and treatment outcomes. Conduct wound care procedures including suturing, ostomy care, and dressing changes while maintaining infection control standards. Provide patient education on disease management, medication administration, nutrition, mobility techniques, and safety measures tailored to each individual's needs. Collaborate with interdisciplinary teams including physicians, therapists, social workers, and case managers to coordinate comprehensive home health services. Monitor patients' conditions during visits, recognize signs of acute pain or deterioration, and triage accordingly to ensure timely interventions. Ensure compliance with HIPAA regulations by maintaining strict confidentiality of patient information and accurate medical documentation. Requirements Valid RN (Registered Nurse) license in good standing with the state licensing board. Proven experience in home health nursing with familiarity in Medicare regulations and documentation requirements. Experience working with diverse populations such as seniors in assisted living or nursing homes, individuals with developmental disabilities or dementia care needs. Excellent patient assessment skills combined with physiology knowledge to evaluate symptoms accurately and prioritize urgent needs effectively. Ability to work independently in a fast-paced environment while maintaining attention to detail and adhering to safety protocols such as aseptic technique and infection control measures. Strong experience conducting OASIS and SOC.
