In this vital role, you will coordinate comprehensive patient care plans, ensuring seamless transitions from hospital to home and promoting optimal health outcomes. This position offers an exciting opportunity to make a meaningful difference in patients' lives by providing compassionate, expert nursing care within their home environment. Duties Manage field staff and coordinate patient care as a nurse case manager by coordinating services across multiple healthcare providers. Utilize electronic health records (EHR) management systems to document patient progress accurately and efficiently. Develop and implement discharge planning strategies that align with hospital medicine, outpatient care, and post-acute needs. Knowledge of health regulation policies including HIPAA compliance and medical records management standards. Proficiency in medical terminology, physiology knowledge, and diagnostic evaluation techniques essential for accurate patient assessment. Skilled in managing patients through intake processes and assisting field staff as needed by creating and faxing orders to physicians for approval. Experience with electronic health records (EHR) systems for documentation of medical records and clinical notes. Strong background in managing diverse patient populations such as pediatrics, behavioral health patients, or those requiring infectious disease care.
