We're looking for a Preventive Cardiology Nurse Practitioner who thrives in a collaborative environment, loves working directly with patients in long-term care settings, and takes pride in delivering proactive, life-changing care. - Your Role Conduct in-person, preventive cardiology and chronic disease-focused visits in Assisted Living and Skilled Nursing settings Monitor patients' chronic cardiac conditions daily, using remote technology Collaborate with primary care providers, other cardiologists, and our internal team.
Document clinical notes and treatment plans with attention to detail Educate patients, families and staff on heart health, prevention, and wellness Contribute to a culture of excellence and patient-centered care What You Bring
- The Ideal Candidate, Active Nurse Practitioner License (Florida).
Strong background or interest in Cardiology or Geriatrics Experience, working in long-term care settings preferred. Proactive care. Integrity, autonomy, and respect. Excellent communication and collaboration skills. Tech-savvy and comfortable using remote monitoring tools. Organized, detail-oriented, and passionate about prevention. Willingness to travel locally between facilities
