$35.04•$47.30 / hour Summary Responsible for providing team leadership for the assessment, triage, care coordination and coordination of the disease education to improve health outcomes for specific moderate to high risk based patient population and wellness activities for low risk patients. Works collaboratively with high risk, value based patient populations. Connects with the patients in person (through home visits), on the phone, on-site in the physician offices, hospital, SNF, and other sites of care. Provides face to-face interaction with patients and their care team when appropriate to improve patient care in person or via telehealth or video visits as needed. Requirements Educational Requirements Degree/Diploma Obtained Program of Study Required/ Preferred and/or Bachelor's Master'sNursing NursingRequired Preferred Experience Requirements Minimum Years Required Area of Experience Required/ Preferred and/or 3 YearsClinical NursingRequired State of Florida Licensure Requirements Licenses Required/ Preferred and/or Registered Nurse LicenseRequired Certifications/Registration Requirements Certificates/Registrations Required/ Preferred and/or ACM (Accredited Case Manager). CCM (Certified Case Manager),NACCM (National Academy of Certified Care Managers) or CMCN (Certified Managed Care Nurse) Preferred Other Requirements Exceptional organization and interpersonal skills. Leadership, education, and counseling skills. Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal and nonverbal. Ability to work well with people of various ages, backgrounds and ethnicities. Ability to work collaboratively in a team and manage multiple priorities using sound administrative and clinical judgment. Proficiency in communication technologies (email, cell phone, etc.)
