Job Description

RN Case Manager We are seeking a compassionate and detail-oriented RN Case Manager to join a leading acute care hospital in Fort Walton Beach, FL. This is an excellent opportunity to support patients through comprehensive care coordination, discharge planning, and collaboration with interdisciplinary teams to ensure high-quality outcomes and appropriate transitions of care.
Shift Options:
Monday•Friday, 8:00am•4:30pm + weekend rotation
Compensation:
$34.25•$47.10/hour Relocation Assistance Available Benefits for

RN Case Manager:
RN License Minimum 2 years of RN experience in an acute care setting (ICU, ER, CCU, Med/Surg, or LTAC Case Management experience) BLS Daily Responsibilities for
RN Case Manager:
Coordinate patient care across the continuum, including assessment, discharge planning, utilization review, and collaboration with interdisciplinary teams to ensure appropriate level of care and timely transitions Advocate for patients by facilitating communication between providers, patients, families, and payers while maintaining accurate, compliant clinical documentation Ready to take the next step?
