Aviata Health Group is seeking a knowledgeable and detail-oriented MDS RN to join our clinical team. The MDS RN plays a critical role in ensuring accurate resident assessments, care planning, and regulatory compliance, while supporting quality outcomes and appropriate reimbursement.Key Responsibilities:
Complete and coordinate the Minimum Data Set (MDS) assessments in accordance with federal and state regulations Ensure accuracy and timeliness of all MDS submissions Coordinate interdisciplinary team (IDT) participation in the assessment and care planning process Develop, review, and update resident care plans based on MDS findings and clinical assessments Monitor case mix index (CMI) and support optimization of reimbursement Ensure compliance with Medicare, Medicaid, and other payer requirements Participate in utilization review and quality assurance performance improvement (QAPI) activities Collaborate with nursing staff, therapy, and physicians to ensure appropriate documentation Identify and address potential coding or documentation issues Maintain up-to-date knowledge of regulatory changes and MDS guidelines Assist with audits and surveys as neededQualifications:
Active Registered Nurse (RN) license in good standing Previous MDS experience in a skilled nursing or long-term care setting preferred Strong knowledge of RAI Manual, Medicare, and Medicaid guidelines Excellent assessment, analytical, and documentation skills Strong communication and team collaboration abilities Ability to manage multiple deadlines and priorities Experience in Skilled Nursing/Rehabilitation facilities preferred.
