Job Requirements :

Associate degree minimum education. Current and unrestricted Florida Registered Nurse license. Minimum of one year of direct patient care experience preferred. Previous home health experience preferred. Experience completing Start of Care visits and OASIS documentation preferred. Clinical assessment, communication, and documentation skills. Ability to work independently and manage assigned visits efficiently. Current CPR certification. Valid driver's license, reliable transportation, and current automobile insurance. Ability to successfully complete all required background screening and onboarding requirements.

Job Responsibilities :

Complete Start of Care assessments and comprehensive patient evaluations. Perform skilled nursing visits, follow-up visits, resumptions of care, recertifications, and discharges, as assigned. Develop and update individualized plans of care. Assess patients' conditions and report significant changes to the appropriate clinical team. Provide medication management, wound care, disease education, and other skilled nursing services. Educate patients and family members regarding care needs, medications, safety, and treatment plans. Communicate and coordinate with physicians, caregivers, therapists, and members of the interdisciplinary care team. Follow agency policies, professional nursing standards, and applicable state and federal regulations Travel to patients' homes throughout the assigned coverage area.

Salary:

$35.00 - $120.00 ( Depends on Experience)
