Job Description Help for Job Description. Opens a new window. Minimum requirements : Associate's degree in Nursing required. Active Florida Registered Nurse (RN) license or Licensed Practical Nurse (LPN) license required. Minimum of one year of nursing experience preferred. Home health, home care, cardiac care, or skilled nursing experience is preferred. RN candidates with Start of Care (SOC) and OASIS documentation experience are preferred. Candidates must have clinical assessment, communication, and documentation skills, be able to work independently in the field, possess reliable transportation, and demonstrate dependability, organization, and a patient-focused approach.
Job Duties :
Provide skilled nursing visits to patients in their homes, including support for cardiac program patients through monitoring of symptoms, condition changes, and overall health status. Complete Start of Care (SOC) visits, nursing assessments, routine visits, and follow-up care in accordance with patient needs and care plans. Deliver medication education, disease management support, patient and family education related to cardiac care, safety, and wellness. Monitor vital signs, communicate concerns to the care team, maintain accurate and timely clinical documentation, and coordinate care with patients, families, physicians, office staff, and clinical team members. Maintain professional, compassionate, and reliable standards of patient care.
Schedule :
Flexible scheduling options available. Full-time, part-time, and per diem opportunities are offered. Service areas include Kissimmee, Orlando, Osceola County, Orange County, and surrounding Central Florida locations.
Wage :
$30 to $40 per hour. Depending on experience.
