What You'll Do Complete comprehensive patient assessments and develop individualized care plans. Provide skilled nursing care in patients' homes according to physician orders and care plans. Monitor patient health conditions and promptly communicate any changes to the healthcare team. Administer medications, treatments, and other nursing interventions as prescribed. Collaborate with caregivers and family members to ensure continuity of care. Educate patients and families on disease management, medication compliance, and preventive health practices. Maintain accurate documentation and ensure compliance with all healthcare regulations and standards. Qualifications Active and valid Florida Registered Nurse (RN) License . Minimum 3• years of nursing experience• (home health experience preferred). Strong assessment, care planning, and clinical decision-making skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively. Valid driver's license and reliable transportation for patient visits.
