Job Description:
We are looking for a reliable Registered Nurse who provides skilled nursing care to patients in their homes, including assessments, medication management, wound care, patient education, and coordination with physicians and care teams. Develops and updates care plans, monitors patient progress, and ensures compliance with healthcare regulations while promoting patient comfort, safety, and independence. Responsibilities Provide direct patient care, including administering medications, performing procedures, and monitoring patient vital signs. Collaborate with physicians and other healthcare professionals to develop and implement patient care plans. Offer health coaching to patients, providing guidance on healthy lifestyle choices and disease prevention. Manage durable medical equipment (DME) for patients in need of assistive devices. Utilize electronic health record systems such as Alora Plus to document patient information accurately.
Qualifications:
Valid Registered Nurse (RN) license in the state of practice. Prior experience in Home Health is a plus. Strong knowledge of healthcare practices, procedures, and regulations. Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members. Ability to prioritize tasks and manage time effectively in a fast-paced environment.
