• Home visits in assigned service area:
• Largo, St. Petersburg, Pinellas Park surrounding areas assigned by zip code. The Home Health Field, RN provides
• hands-on care
• in the home for a group of patients consistently using the nursing process and clinical standards and protocols.
• Responsibilities include
• : + Oversee the activities of the multi-disciplinary team that provides care to the patient + Monitor the provision of therapeutic services through written and verbal communication with all disciplines + Ensure that all visits scheduled and provided are authorized and covered by physician orders + Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits + Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor •

Home Health RN Requirements:

• + BLS (Basic Life Support) + RN (Registered Nurse) License + Driver's License State of Florida (Florida driver's license is required within 30 days of hire) + Required Associate's or Diploma Nursing + Required 1 year Registered Nurse + _Preferred_ Bachelor's Nursing + _Preferred_ 1 year Home Care
