Job Description
JOB DETAILS POSITION SUMMARY
- RN Case Manager
POSITION DUTIES
- Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient's medical needs and recovery goals.
MINIMUM REQUIRED QUALIFICATIONS
- FL/compact RN license BLS 2+ years minimum of acute care hospital experience in specialty Acute hospital experience in specialty during the past year with no more than a 90-day break Meditech & Midas experience
PREFERRED QUALIFICATIONS
- Complex discharge planning experience
