$1800.00•$2700.00 POSITION SUMMARY•RN Case Manager
POSITION DUTIES
•Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization
MINIMUM REQUIRED QUALIFICATIONS
•FL/compact RN license;
BLS:
2+ years minimum of acute care hospital experience in specialty; Acute hospital experience in specialty during the past year with no more than a 90-day break
LENGTH OF ASSIGNMENT
•13 weeks
SHIFT / HOURS PER WEEK
•8:00am-4:30pm M-F and every other weekend SYSTEMS•Meditech, Midas, Wellsky
