Admissions Nurse

Admissions Nurse The Admissions Nurse is responsible for coordinating and managing the clinical admissions process to ensure appropriate placement, timely intake, and regulatory compliance.

Key Responsibilities:

Review and clinically screen referrals for SNF appropriateness Coordinate admissions with hospitals, case managers, and internal teams Complete and verify admission paperwork, assessments, and authorizations Ensure compliance with CMS, Medicare, Medicaid, and facility policies Conduct admission assessments and support smooth resident transitions Communicate clearly with residents, families, and interdisciplinary teams

Qualifications:

RN License in the state of Florida SNF or post‑acute admissions experience preferred Knowledge of Medicare criteria, PDPM, and insurance authorizations Strong clinical judgment, organization, and communication skills
