We're a small, close-knit, RN-led team that does home health the way it should be done — consistent caregivers, fast and reliable starts of care, and real clinical oversight behind every visit. We're growing, and we're looking for a skilled Home Health RN who wants to deliver high-quality care without the burnout of a packed hospital floor. What You'll Do Provide skilled nursing visits to patients in their homes across Lee and Collier County Complete assessments, develop and follow the plan of care, and document accurately in our EMR (WellSky/Kinnser) Perform Start-of-Care and OASIS documentation Administer treatments, wound care, medication management, and patient/family education Coordinate with our RN-led clinical team, physicians, and therapists to keep care on track Help our patients recover safely and stay independent at home What We're Looking For Active, unrestricted Florida RN license 1+ year of home health experience OASIS / Start-of-Care experience preferred Current CPR/BLS certification Valid Florida driver's license, reliable transportation, and auto insurance Strong assessment skills, solid documentation, and a heart for patient-centered care Bilingual (Spanish or Haitian Creole) is a plus, not required What We Offer Per-visit compensation with flexible PRN / as-needed scheduling — you control your availability A supportive, RN-led clinical team that has your back Consistent patient assignments through our matched-caregiver model A faith-based mission and a workplace culture that treats both patients and staff like family The chance to grow with an agency that's scaling the right way Background Screening This position requires a Level 2 background screening through the Florida Care Provider Background Screening Clearinghouse.
