Job Description

Moorings Park Communities is seeking a MDS Coordinator. The MDS Coordinator coordinates the appropriate participating health professionals for the purpose of conducting initial and periodic comprehensive, accurate assessments of each resident to plan the care that allows the resident to reach his/her highest practicable level of physical, mental, and psychosocial functioning. Also, conducts and coordinates the completion of the resident comprehensive assessment which includes the Minimum Data Set (MDS).
Your contributions:
Organizes and conducts the care plan process and conference with nursing staff, ancillary departments, residents, and families. Completes
MDS/PDPM
requirements according to State and Federal guidelines. Updates individualized care plans quarterly and on an as needed basis. Maintains the care plan calendar and distributes to all appropriate personnel. Participates in the weekly Medicare Meeting to review all residents for continued compliance with Medicare A classification. Transmits required
MDS/PDPM
documents to the appropriate authority on a timely basis and as needed. Assures all appropriate signatures are present on all care plan documents. Oversees the monitoring and documentation of the residents' status in numerous physical, behavioral, activities of daily living, and treatment areas. Participates in staff education and training in relation to accurate documentation. Works with interdisciplinary staff to keep families of residents informed of and involved in residents' care. Demonstrates clinical assessment skills at the level necessary to meet the job requirements. Maintains current clinical knowledge and skills through current literature, conferences, workshops and seminars, as well as knowledge of payment systems in long term care.
JOB REQUIREMENTS
Associate degree or bachelor's degree in nursing from an accredited program required Active Florida Registered Nurse licensure required Active Florida CPR Certification is required During emergencies or natural disasters may be required to be on duty for the duration of the disaster. Excellent oral and written communication skills in the English language required. Minimum 6 months experience medical-surgical nursing preferred. Previous experience with geriatric patients desirable.
