Job Description
RN Field Case Manager We're looking for a Registered Nurse who enjoys being out in the field while still having support from a small, collaborative office team. This role is mostly field-based, with occasional time in the office as needed. If you like building relationships with clients and caregivers while helping ensure quality care, this could be a great fit. What you'll be doing:
- Oversee and coordinate non-skilled home care cases
- Act as the main point of contact for clients, caregivers, and referral partners
- Help with day-to-day office needs like scheduling, authorizations, and client communication
- Conduct caregiver orientations, training, and supervisory visits in the field
- Keep an eye on compliance with state regulations and internal standards
- Work alongside leadership to improve client satisfaction and overall care quality What we're looking for:
- Active RN license in Florida
- At least a year of home care or community health experience is a plus Strong communication and organizational skills
- Someone who's dependable, professional, and comfortable working independently
- Valid driver's license and reliable transportation
- Experience with home care software is helpful but not required What you can expect:
