Full Time Home Health RN

Overview We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team in Brevard county, Fl. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background and the ability to work independently while maintaining effective communication with patients, families, and healthcare teams. Responsibilities Conduct comprehensive patient assessments to develop individualized care plans. Administer medications and treatments as prescribed by physicians. Monitor patient progress and adjust care plans as necessary. Provide education and support to patients and their families regarding health management and disease prevention. Collaborate with interdisciplinary teams to coordinate care and ensure optimal patient outcomes. Document all patient interactions, assessments, and interventions accurately in electronic health records. Assist with pain management strategies and provide urgent care when needed. Utilize knowledge of ICD-9 coding for accurate documentation of diagnoses. Maintain compliance with all regulatory standards and best practices in home health care. Requirements Current Registered Nurse (RN) license in the state of practice. Previous experience in skilled home care. 1 year Medicare OASIS Home Care required. Experience working Wellsky is a plus. Excellent communication skills and the ability to build rapport with patients and families. Strong organizational skills and the ability to manage multiple tasks effectively.
