Do:
As a valued member of our interdisciplinary team, you will: Deliver compassionate, patient-centered care aligned with our mission, vision, and values Coordinate care for patients and families under the direction of the primary physician Assess patient and family needs , ensuring holistic support (physical, emotional, and spiritual) Develop and implement individualized care plans as part of the Interdisciplinary Group (IDG) Ensure seamless delivery of services and interventions outlined in the care plan Prioritize comfort and dignity , making each patient and family interaction meaningful Provide personalized, sincere, and engaging service at all times Promptly address concerns to ensure patient and family satisfaction Uphold the highest standards of confidentiality and respect What You'll Need Registered Nurse currently licensed in the State of Florida. BSN preferred Hospice certification preferred. Current Basic Life Support Certification. One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred. Understanding of and ability to apply characteristics of Hospice Nursing:
case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs Basic computer skills and EMR software experience a plus.
