Job Description

Overview Join our dynamic healthcare team as a Registered Nurse (RN) specializing in Home Care, where your expertise will make a meaningful difference in patients' lives. In this vital role, you will deliver comprehensive nursing care directly in patients' homes, ensuring they receive personalized, high-quality treatment that promotes recovery, independence, and well-being. Your clinical skills and compassionate approach will support a diverse patient population across various settings, including post-acute care, outpatient clinics, and community health environments. This position offers an exciting opportunity to work autonomously while collaborating with multidisciplinary teams to optimize patient outcomes. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to develop individualized care plans. Provide skilled nursing interventions such as wound care, medication administration, IV therapy, and disease management tailored to each patient's needs. Coordinate discharge planning and transition of care by communicating with hospitals, primary care providers, and specialists to ensure seamless patient handoffs. Educate patients and their families on health management strategies, medication adherence, and lifestyle modifications to promote recovery and prevent readmissions. Monitor patient progress through ongoing evaluation and documentation in medical records while adhering to HIPAA regulations for confidentiality. Manage acute care situations effectively by recognizing signs of deterioration and initiating appropriate interventions or emergency responses. Collaborate with case managers and utilization review teams to optimize resource use and ensure compliance with managed care protocols. Experience Candidates should possess a robust background in various clinical environments such as critical care units—including ICU (Intensive Care Unit), PICU (Pediatric Intensive Care Unit), emergency rooms, or trauma centers—demonstrating proficiency in acute and post-acute care delivery. Experience working within Level I or Level II trauma centers is highly valued, along with familiarity in hospital medicine, infectious disease management, behavioral health settings, geriatrics, and medical-surgical units. Prior outpatient or clinic experience is beneficial for understanding diverse patient populations across inpatient and outpatient settings. A strong foundation in physiology knowledge, medical terminology, diagnostic evaluation, discharge planning, and case management is essential. Candidates should also have experience with intake processes, utilization management strategies, inpatient/outpatient care coordination, post-acute services, and outpatient clinic operations. Demonstrated ability to perform comprehensive patient assessments while maintaining compliance with HIPAA standards is critical for success in this role.
