Job Description
Suncoast Hospice , a member of Empath Health is seeking a caring and motivated RN Case Manager to work with us in North Pinellas in while providing exceptional care. Since 1977, Suncoast Hospice has been honored to serve our neighbors across Pinellas and Hillsborough counties with compassionate end-of-life care. As part of Empath Health, we help those living with advanced illness find comfort, dignity, and meaning, while supporting their families and caregivers with guidance and care every step of the way. What you'll Do • is responsible for identifying and coordinating patient/family needs and for providing preventative and supportive care to the terminally ill patient/family unit. • utilizes assessment, intervention and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.
