Job Description

Suncoast Hospice , a member of Empath Health is seeking a caring and motivated APRN to work on call while providing exceptional care. This is an on-call position with duties beginning at 5pm and ending at 8am the following morning Monday through Friday and then from 5pm Friday to 8am Monday. The schedule is very other week with 7 days on and 7 days off. Since 1977, Suncoast Hospice has been honored to serve our neighbors across Pinellas and Hillsborough counties with compassionate end-of-life care. As part of Empath Health, we help those living with advanced illness find comfort, dignity, and meaning, while supporting their families and caregivers with guidance and care every step of the way. What you'll Do This position has responsibility for taking 1st call for hospice patients enrolled within Hospice Programs. They will assess the needs of the patient and determine/communicate next steps as it relates to plan of care for the patient. Identifies, implements, and evaluates appropriate plans of care, including pharmacologic and non-pharmacologic interventions within the guidelines of the Florida APRN Practice Act. Promotes positive health behaviors and self-care skills through education and counseling. Provides accurate and appropriate documentation of patient/family services as specified in policies and submits in a timely manner. Documents orders given in patient's chart. Takes calls originating from home and facility evening/weekend teams for symptom management of hospice patients. Identifies, implements, and evaluates appropriate plans of care, including pharmacologic and non-pharmacologic interventions within the guidelines of the Florida APRN Practice Act. What You'll Need Education:
Master's Degree in Nursing or a related field.
Licensure:
Current Florida license as an Advanced Registered Nurse Practitioner, certified in a practice are (if applicable to departmental requirements). Current DEA (Drug Enforcement Agency) license.
Certifications:
Valid CPR certification from the American Red Cross or American Heart Association. NPI provider and ability to bill CMS services or ability to obtain same (if applicable to departmental requirements). Current DEA License. Minimum of two (2) years nursing experience required. Minimum of two (2) year experience as an APRN required, with experience in hospice, gerontology, or oncology practice preferred.
