The ideal candidate will play a critical role in coordinating patient care, ensuring that patients receive the necessary medical attention throughout their treatment journey. This position requires a strong understanding of clinical practices and an ability to work collaboratively with various healthcare professionals in a fast-paced environment. Duties Assess and evaluate patients' needs for care and develop individualized care plans. Coordinate with multidisciplinary teams, including physicians, social workers, and therapists, to facilitate optimal patient outcomes. Manage discharge planning processes to ensure safe transitions from hospital to home or other care settings. Maintain accurate medical records and documentation for all patient interactions and care plans. Provide education and support to patients and their families regarding treatment options, resources, and follow-up care. Monitor patient progress and adjust care plans as necessary based on ongoing assessments. Collaborate with managed care organizations to ensure compliance with insurance requirements and facilitate authorization processes. Address behavioral health needs as part of comprehensive patient assessments. Requirements Current Registered Nurse (RN) license in the state of practice. Experience working in a Level I or Level II trauma center or emergency room preferred. Knowledge of critical care practices and discharge planning processes. Familiarity with managed care systems and medical records management. Strong understanding of physiology and behavioral health principles. Excellent communication skills, both verbal and written, with the ability to work effectively in a team-oriented environment. Ability to handle multiple priorities while maintaining attention to detail in a fast-paced setting. Previous experience in case management is highly desirable.
