00 per hour Make a Difference in Resident Care Charlotte Bay Rehab and Care Center is seeking a knowledgeable and detail-oriented MDS Coordinator (RN/LPN) to oversee the Resident Assessment Instrument (RAI) process within our skilled nursing facility. This role is essential to ensuring accurate resident assessments, regulatory compliance, optimal reimbursement under PDPM, and the delivery of exceptional resident-centered care. Key Responsibilities Complete and coordinate all MDS assessments in compliance with federal and state regulations. Develop, implement, and maintain individualized care plans based on assessment outcomes. Collaborate with the interdisciplinary team to ensure continuity and quality of care. Monitor PDPM requirements and ensure documentation supports accurate Medicare reimbursement. Review Quality Indicator (QI) and Quality Measure (QM) reports and implement corrective actions when necessary. Participate in state surveys, audits, and regulatory inspections. Ensure timely scheduling, completion, and electronic submission of MDS assessments. Educate and support clinical staff regarding MDS, PDPM, and regulatory updates. Maintain accurate, organized, and compliant documentation practices. Qualifications Active Florida RN or LPN license required. Minimum of two (2) years of skilled nursing facility experience preferred. Strong knowledge of MDS 3.0, the RAI process, and care planning. Experience with PDPM and Medicare documentation required. Excellent organizational, communication, and leadership skills. Ability to work independently while collaborating effectively with an interdisciplinary team.
