Job Description Summary:
Provides comprehensive primary, acute, and extended care services in accordance with scope of practice, licensure, and the Collaborative Practice Agreement. Conducts assessments, diagnoses health conditions, implements clinical interventions, and evaluates patient responses to care. The role supports clinical operations by coordinating patient care in collaboration with physicians and interdisciplinary team members while ensuring compliance with organizational policies and regulatory requirements. Delivers exceptional customer service and contributes to a respectful, team-based care environment.
Knowledge, Skills, and Abilities:
Abilities in the use of specialized equipment used in respective area of practice Required Strong interpersonal skills, excellent conflict resolution, and communication skills Required Leadership/mentorship/clinical resource skills Required Ability to multi-task on a variety of tasks and projects Required Excellent organization skills Required Skilled in providing age-appropriate education regarding care management interventions and illness adjustment issues
