Job Description

Trustbridge Hospice, a part of Empath Health is seeking a caring and motivated RN to provide telephonic support in our West Palm Beach office while providing exceptional care on the following schedule: Per Diem 7am - 7:30pm Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness. What you'll Do Collaborate with departments and staff throughout the organization to deliver coordinated, high-quality patient services. Promptly and efficiently respond to patient-related calls, ensuring needs are addressed in a timely and professional manner. Provide quality, professional telephonic intervention to patients in accordance with the Plan of Treatment. Address patients' physical, emotional, and psychological needs through empathetic communication and appropriate support. Maintain adherence to scope of practice, organizational policies, and applicable regulatory standards. Join a mission-driven team dedicated to kindness, compassion, and Full Life Care for All. What You'll Need Registered Nurse currently licensed in the State of Florida. Current Basic Life Support Certification. Two years in home care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing. Possesses extensive knowledge of professional Nursing principles, practices, and procedures necessary to assess the needs of patients. Understanding of and ability to apply characteristics of Hospice nursing, case management and crisis intervention. EMR software experience.
