Available on a case-by-case basis Overview Join a Level II Trauma Center as a Case Manager RN supporting patients across all inpatient service lines. This role coordinates discharge planning, care transitions, resource utilization, and patient advocacy while collaborating with physicians, nursing leadership, rehabilitation teams, and community providers. This opportunity is ideal for experienced acute care case managers seeking a stable weekday schedule, strong interdisciplinary support, and comprehensive benefits.
