PM Pay:
$36.96-$44.00/hour
Weekend Differential:
$1.50/hour
Sign-On Bonus:

Available on a case-by-case basis Overview Join a Level II Trauma Center as a Case Manager RN supporting patients across all inpatient service lines. This role coordinates discharge planning, care transitions, resource utilization, and patient advocacy while collaborating with physicians, nursing leadership, rehabilitation teams, and community providers. This opportunity is ideal for experienced acute care case managers seeking a stable weekday schedule, strong interdisciplinary support, and comprehensive benefits. Responsibilities Coordinate discharge planning and transitions of care for hospitalized patients Complete psychosocial assessments and discharge planning evaluations Develop individualized care plans based on patient needs and goals Educate patients, families, and caregivers regarding treatment plans and available resources Collaborate with physicians, nurses, therapists, and interdisciplinary teams Facilitate family meetings and care conferences Identify barriers to discharge and arrange appropriate post-acute services Ensure accurate documentation and regulatory compliance Support quality outcomes, patient satisfaction, and efficient resource utilization Qualifications Required Active RN license Associate Degree in Nursing or Nursing Diploma Minimum 2 years of recent acute care Case Management RN experience within the last year Strong knowledge of discharge planning, utilization management, and care coordination Preferred BSN CCM or ACM certification Experience in trauma, cardiac, rehabilitation, or medical-surgical settings Compensation & Benefits Competitive hourly compensation Weekend differential pay Medical, dental, and vision insurance Behavioral health and telemedicine services 401(k) with company match Employee Stock Purchase Plan Paid time off and paid family leave Tuition assistance and student loan support Certification reimbursement and career development programs Life and disability insurance Fertility, adoption, and family-building benefits Ideal For Experienced acute care case managers who enjoy discharge planning, care coordination, patient advocacy, and interdisciplinary collaboration in a hospital setting.
