Days with Rotating Weekends Shift Details Monday-Friday with rotating weekends (every other Saturday; may transition to every third Saturday once fully staffed).

• 7:30 PM Position Overview We are seeking an experienced RN Case Manager to support patients and families throughout their healthcare journey.

This role focuses on care coordination, discharge planning, patient advocacy, resource management, and collaboration with interdisciplinary healthcare teams to ensure quality patient outcomes and smooth transitions of care. Key Responsibilities Coordinate patient care plans and discharge planning activities. Conduct psychosocial assessments and evaluate patient needs. Educate patients, families, and caregivers regarding treatment plans, discharge goals, patient rights, and available resources. Assist with care coordination across the continuum of care. Collaborate with physicians, nursing staff, rehabilitation teams, and department leadership. Participate in family conferences and treatment planning discussions. Ensure accurate documentation and communication of patient care needs. Support safe and effective transitions from hospital to post-acute settings. Required Qualifications Active Registered Nurse (RN) License or APRN License. Associate Degree in Nursing or RN Diploma. Minimum 2 years of recent Acute Care RN Case Management experience. Recent Case Management experience must be within the past 12 months.

All inpatient hospital units Compensation Hourly Pay Range $36.96/hr

• Entry Level $40.48/hr
• Mid-Level (5-6 years of experience) $44.00/hr
• Experienced Level (10-17 years of experience)

Additional Pay Weekend Differential:

$1.50/hr

Additional Incentives Sign-On Bonus:
