- RN•Case Manager Location Sanford, Florida Schedule / Job Type Contract | Travel | Days | 9:00 AM•5:30 PM | 12-Week Assignment Key Responsibilities • Perform DCP assessments and apply InterQual criteria to support utilization review and care decisions• Coordinate transitions of care including home health and skilled nursing facility placements• Round with multidisciplinary teams, physicians, and interdisciplinary team (IDT) members• Manage acute care case management functions including utilization review, discharge care planning, and transitions of care• Document accurately and efficiently using Meditech and MIDAS systems
