  • Acute Care Seeking an experienced RN Case Manager to join a busy acute care hospital in the Sanford, FL area.
Schedule:
Days with rotating weekends Shift times may vary: 8:00 AM
  • 4:30 PM 9:00 AM
  • 5:30 PM 10:00 AM
  • 6:30 PM 11:00 AM
  • 7:30 PM Every other Saturday required, potentially every third Saturday once fully staffed.
What You'll Do:
Coordinate patient care and discharge planning across inpatient units Support patients and families throughout the continuum of care Develop individualized treatment and discharge plans Complete psychosocial assessments and collaborate with interdisciplinary teams Educate patients, caregivers, and families on treatment goals and available resources Advocate for safe, patient-focused outcomes
What We're Looking For:
Active RN or APRN license Associate Degree or RN Diploma required Minimum 2+ years of recent acute care Case Management RN experience Strong communication, critical thinking, and organizational skills Comfortable collaborating with physicians and hospital leadership in a fast-paced environment
Facility Highlights:
221-bed acute care hospital Level II Trauma Center Full-service cardiovascular program including open-heart surgery and interventional cardiology Orthopedic, spine, robotic surgery, and inpatient rehab services
