PM Position Overview:
Navitas Healthcare is seeking an experienced Case Manager RN to support patients and families throughout their healthcare journey. The ideal candidate will coordinate care, facilitate discharge planning, connect patients with community resources, and collaborate with interdisciplinary teams to ensure safe and effective transitions of care. Responsibilities Guide patients and families through program orientation and care planning processes. Educate patients, caregivers, and families regarding treatment goals, discharge planning, and available resources. Develop and implement individualized patient care and discharge plans. Conduct psychosocial assessments and document findings accurately. Coordinate services across multiple departments to ensure continuity of care. Collaborate with physicians, nursing staff, rehabilitation teams, and leadership. Advocate for patient needs while promoting quality outcomes and patient satisfaction. Support utilization management and care coordination activities. Facilitate safe and efficient patient transitions across the continuum of care. Maintain accurate and timely documentation in accordance with organizational standards. Required Qualifications Active Registered Nurse (RN) License or APRN License required. Associate Degree in Nursing (ADN) or RN Diploma required. Minimum 2 years of recent Acute Care Case Management RN experience within the last year required. Strong discharge planning, utilization management, and care coordination experience. Experience working in an acute care hospital setting. Strong communication and interdisciplinary collaboration skills. Ability to perform patient assessments and coordinate complex care plans. Knowledge of healthcare regulations, payer requirements, and community resources. Strong organizational, documentation, and problem-solving skills. Ability to manage multiple priorities in a fast-paced healthcare environment.
