Home Health RN

The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes in accordance with physician orders and the established plan of care. The RN is responsible for patient assessments, care coordination, education, and clinical documentation to promote optimal patient outcomes and safety. Responsibilities Conduct initial, resumption, recertification, and discharge assessments Develop, implement, and update plans of care in coordination with physicians and the interdisciplinary team Perform skilled nursing visits , including wound care, medication management, IV therapy (as applicable), injections, catheter care, and disease management Educate patients and caregivers on diagnoses, medications, treatments, and safety Accurately document visits and patient progress in the EMR per agency and regulatory standards Communicate changes in patient condition promptly to the physician and office team Ensure compliance with Medicare, state, and agency policies and procedures Participate in case conferences and quality improvement initiatives Supervise and collaborate with LPNs, HHAs, and other clinicians as required Requirements Active Registered Nurse (RN) license in the state of employment Minimum 1 year of RN experience (home health experience preferred) Current CPR/BLS certification Valid driver's license , reliable transportation, and auto insurance Ability to pass background screening and health requirements (TB/physical as required) Strong clinical assessment, communication, and documentation skills Ability to work independently in the field Preferred Qualifications Prior home health or Medicare-certified agency experience Experience with OASIS and home health EMR systems Bilingual a plus Familiarity with local referral sources and community resources Schedule & Compensation Full-time, part-time, or per diem opportunities available Flexible scheduling Competitive pay based on experience and visit type Work Environment Field-based position providing one-on-one patient care in the home Supportive clinical and administrative team
