: 12 hour shifts with Great Benefits! Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic team at a leading Skilled Nursing Facility. In this vital role, you will provide high-quality nursing care to residents, ensuring their comfort, safety, and well-being. Your expertise will support the interdisciplinary team in delivering personalized care plans, managing complex health conditions, and promoting optimal recovery and quality of life. Shifts:
12hrs 7a-7p or 7p-7a Responsibilities Deliver comprehensive nursing care to residents, including medication administration, wound care, and vital signs monitoring. Conduct thorough patient assessments, document medical histories, and update electronic health records (EHR) systems accurately. Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans. Monitor residents for signs of acute pain or deterioration, providing timely interventions and triage as needed. Educate residents and their families on health management strategies, medication safety, and discharge planning. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN). Proven experience in long-term care settings such as nursing homes or assisted living facilities. Experience with EMR/EHR systems like Matrix is highly desirable. Demonstrated competence in critical care procedures including ventilator management, infection control practices, and acute pain management. Ability to perform physical examinations, vital signs assessment, specimen collection/processing, and basic life support (BLS). Excellent communication skills with the ability to work effectively with individuals with disabilities or cognitive impairments such as Alzheimer's disease or dementia.
