Job Description

Hometown Care is expanding! We are a private care company seeking an experienced and highly skilled Registered Nurse (RN) to join our growing, family-owned agency based in Stuart, Florida! Founded by a Fire Rescue Lieutenant Paramedic and rooted deeply in the local community, we pride ourselves on delivering high-quality, personalized care with integrity and heart.
Pay:
Competitive salary- based position that is based on experience and qualifications and will be discussed during interview process This is an in-office role ideal for an RN with a strong background in private duty and skilled home care, who thrives in a collaborative environment and is passionate about elevating care standards.
Position Overview:
The Registered Nurse will play a key role in overseeing all clinical aspects of our private duty services, including conducting client assessments, developing care plans, and ensuring appropriate oversight of skilled cases. This position works closely with our office team to support both clients and caregivers while maintaining compliance with all applicable regulations.
Key Responsibilities:
Conduct initial and ongoing client assessments for private duty cases Develop and implement individualized care plans Provide clinical oversight for skilled private care services Supervise and support caregivers to ensure quality and consistency of care Collaborate with physicians, hospice teams, and other healthcare professionals Monitor documentation and ensure compliance with Florida regulations Provide guidance and education to caregivers and families as needed Assist in case management and coordination of services
Qualifications:
Active Florida Registered Nurse (RN) license (required) Minimum 2+ years of private duty home care experience (required) Skilled care experience in a home setting (required) Strong knowledge of home care regulations and best practices Excellent assessment, communication, and organizational skills Ability to work in a fast-paced, team-oriented environment Compassionate, professional, and detail-oriented
