What You'll Do:
As an FCN Coordinator, you'll play a vital role in shaping and supporting community-based health initiatives: ? Serve as a liaison between the healthcare system, faith communities, and the broader community ? Support the development, implementation, and evaluation of FCN programs ?⚕️ Recruit, train, and guide paid and volunteer Faith Community Nurses ? Promote holistic care—addressing physical, emotional, and spiritual well-being ? Assess community health needs and coordinate wellness screenings & education events ? Deliver engaging presentations to nurses, clergy, and community groups ?️ Maintain program data and ensure compliance with reporting requirements ? Build strong partnerships with congregations and community organizations ? Coordinate care for patients in the FCN Transitional Care Program ? Travel between facilities and community events as needed ➕ Take on additional responsibilities that support the mission ✅Role Requirements:
Education:
Associate's Degree or Diploma in Nursing (BSN preferred) ?Certifications:
Active RN license + BLS certification ?Experience:
At least 3 years of direct patient care ? Comfortable with Microsoft Office and basic computer skills ? Commitment to growth—complete BLS Instructor Training within 6 months ? Why Join BayCare?
