Job Description:
Collaborates on a plan of care based on patient needs and considerations, involving patients and/or others in the process. Evaluates and modifies the plan of care based on patient responses and attainment of expected outcomes, including conducting medication reconciliation. Adheres to established standards of practice, demonstrating accountability for actions and adherence to hospital policies and procedures. Functions as a core member of the care team, providing leadership, organizing program materials, and assisting in efforts to promote care coordination. Ensures timely communication with patients, significant others, and other health care providers through professional collaboration. Collaborates with other health care providers to deliver patient-centered care coordination in a manner consistent with safe, efficient, and cost-effective resource utilization. Plans, implements, and documents patient visits, ensuring timely and appropriate chart documentation. Assists with community outreach educational programs related to care coordination, including the development and distribution of educational materials. Completes direct patient care coordination with the Community Care and Transitional Care Programs, providing support when needed at other locations within the region as directed. Provides patient assessment for medical and behavioral needs, identifying individualized physical, psychological, and emotional outcomes. Formulates a plan of care based on assessment, diagnosis, age, and patient profile, ensuring it is consistent with safe, effective patient care. Other duties as assigned.
Knowledge, Skills, and Abilities:
Professional knowledge of mind-body medicine and current research within the field. [Required] Computer proficiency. [Required]
