Role:

We are seeking a dedicated and detail-oriented MDS Coordinator to join our interdisciplinary team. As a MDS Coordinator , you will be responsible for coordinating the care and services of our residents, working closely with our nursing staff, physicians, and other healthcare professionals to ensure seamless transitions and optimal outcomes. Your expertise in MDS assessments, care planning, and collaboration will be invaluable in helping us achieve our mission to transform ordinary expectations into extraordinary outcomes.

Responsibilities:

Conduct MDS assessments and develop comprehensive care plans in collaboration with the interdisciplinary team Coordinate care services, including wound care, physical, occupational, and speech therapies, and other specialized services Ensure accurate and timely completion of MDS assessments, care plans, and other required documentation Develop and maintain strong relationships with residents, families, and healthcare providers Participate in quality improvement initiatives and contribute to the ongoing development of our care programs Maintain accurate records and reports, ensuring compliance with regulatory requirements and company policies
