Job Description:

Matrix Home Care is currently seeking compassionate and dependable Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to support Start of Care (SOC) and skilled nursing visits in the home care setting in Venice, FL. The Registered Nurse or Licensed Practical Nurse will provide quality patient care in the patient's home while supporting each individualized plan of care. This position is ideal for nurses who enjoy working independently, completing patient visits, and providing hands-on care in a supportive home care environment.

Responsibilities Include:

Complete Start of Care (SOC) visits and skilled nursing visits as assigned Provide skilled nursing care to patients in their homes Complete patient assessments and follow the established plan of care Provide medication management and patient education Assist with wound care, G-tube care, trach care, and other clinical needs as applicable Monitor patient condition and report changes to the appropriate clinical team Complete documentation accurately and timely in the electronic medical record system Communicate with patients, families, caregivers, and clinical leadership Follow all company policies, safety standards, and regulatory requirements

Qualifications:

Active Florida Registered Nurse license or Licensed Practical Nurse license Minimum of 1 year of nursing experience preferred Home care or home health experience preferred Experience with Medicare documentation, Start of Care, routine visits, or skilled nursing visits is a plus Strong clinical assessment and documentation skills Ability to work independently in the field Reliable transportation required Professional, dependable, and compassionate
