Job Description

The Clinical Manager is responsible for the supervision and coordination of all patient care services to ensure the delivery of high-quality, compliant home health care. This position provides clinical oversight, staff support, quality assurance, and care coordination while maintaining compliance with Medicare, AHCA, HIPAA, and agency standards. The Clinical Manager works closely with the Director of Clinical Services (DCS), field clinicians, physicians, referral sources, patients, and families to ensure exceptional patient outcomes and operational excellence. This individual may serve as the Alternate Director of Clinical Services when designated. Essential ResponsibilitiesClinical Oversight & Quality Assurance Review OASIS assessments for accuracy, completeness, and regulatory compliance. Recommend revisions to assessing clinicians as needed and coordinate submission to coding. Review and approve Plans of Care prior to physician submission. Audit clinical documentation to ensure medical necessity, homebound status, and skilled services are appropriately documented. Monitor and manage infection control reports, incident reports, and patient complaints. Ensure compliance with Medicare Conditions of Participation, AHCA regulations, HIPAA requirements, and agency policies. Staff Leadership & Support Provide direct supervision and support to clinical field staff, including RNs, LPNs, HHAs, and therapists. Educate and mentor clinicians to maintain high documentation and patient care standards. Assist with staff scheduling concerns, PTO requests, and clinical staffing issues. Promote teamwork, accountability, and a culture of excellence. Care Coordination & Customer Service Serve as the primary clinical contact for patients, families, physicians, referral sources, and community partners. Collaborate closely with the sales and operations teams to ensure seamless patient care and communication. Address patient and caregiver concerns promptly and professionally. Facilitate effective communication between all members of the interdisciplinary care team. Operational Responsibilities Assist with process improvement initiatives and operational workflow development. Participate in agency meetings, quality improvement activities, and regulatory readiness efforts. Support census growth by ensuring timely and efficient patient care coordination. Perform additional duties as assigned by agency leadership. Qualifications Current Florida Registered Nurse (RN) license required. Minimum two (2) years of Home Health experience preferred. Prior supervisory or leadership experience preferred. Strong knowledge of OASIS, Medicare regulations, AHCA requirements, and home health documentation standards. Excellent communication, organizational, and leadership skills. Proficiency with EMR systems and Microsoft Office applications. Current CPR certification. Valid driver's license and reliable transportation. Work Environment Office-based position with occasional field responsibilities as needed. Not a remote position.
