Matrix Home Care is now hiring experienced Registered Nurses (RNs) to support Start of Care (SOC) and skilled nursing visits in the North Palm Beach, Palm Beach Gardens, and Jupiter areas. We are seeking dependable, compassionate, and professional RNs who are comfortable providing care in the home setting. The ideal candidate will have clinical assessment skills, home health or home care experience, and the ability to work independently while providing high-quality patient care.

Position Summary:

The Registered Nurse will provide skilled nursing services to patients in their homes, including Start of Care assessments, routine nursing visits, follow-up visits, patient education, and care coordination. This role is ideal for an RN who enjoys working in the field, building relationships with patients, and providing meaningful one-on-one care in the home setting.

Responsibilities:

Complete Start of Care visits and skilled nursing assessments Provide routine nursing visits and follow-up care as assigned Assess patient needs and monitor changes in condition Develop, update, and follow patient care plans Provide patient and family education Complete accurate and timely clinical documentation Communicate with physicians, office staff, and the care team regarding patient needs Support care coordination based on patient condition and visit needs Maintain professional, compassionate, and reliable patient care standards

Qualifications:

Active Florida Registered Nurse license required Minimum of 1 year of nursing experience preferred Home health or home care experience preferred Experience with Start of Care and OASIS documentation preferred Clinical assessment, communication, and documentation skills Ability to work independently in the field Must have transportation Must be dependable, organized, and patient-focused
