Job Description

Job Overview We are seeking a dedicated and compassionate Hospice Registered Nurse (RN) Case Manager to provide comprehensive end-of-life care coordination for patients in a hospice setting. The ideal candidate will possess extensive clinical expertise, including critical care, trauma, and acute care experience, to deliver personalized support that enhances quality of life for patients and their families. As a Hospice RN Case Manager, you will be responsible for assessing patient needs, developing individualized care plans, and ensuring seamless communication among interdisciplinary teams. This role offers an opportunity to make a meaningful difference through expert nursing practice combined with compassionate patient-centered care. Responsibilities Conduct thorough patient assessments to evaluate physical, emotional, and psychosocial needs, utilizing skills in vital signs measurement and physiological evaluation. Develop and implement comprehensive care plans tailored to each patient's unique circumstances, incorporating principles of palliative and end-of-life care. Coordinate with healthcare providers across hospital medicine, outpatient clinics, and post-acute settings to ensure continuity of care and appropriate discharge planning. Manage medication administration, symptom management, and diagnostic evaluations while adhering to HIPAA regulations and medical terminology standards. Provide education and support to patients and families regarding disease progression, treatment options, and advance directives. Collaborate with interdisciplinary teams including behavioral health specialists, infectious disease experts, and primary care providers to optimize patient outcomes. Document all patient interactions accurately within medical records systems and participate in utilization management and case management activities to ensure efficient resource utilization. Requirements Valid RN license with current licensure in the state of practice. Proven clinical experience in critical care environments such as ICU, Level I or Level II trauma centers, or emergency room settings. Experience working within hospital medicine or medical-surgical units; post-acute or outpatient clinic experience is advantageous. Strong knowledge of physiology, diagnostic evaluation procedures, discharge planning processes, and inpatient/outpatient care coordination. Familiarity with managed care protocols, utilization management practices, and medical records documentation standards. Demonstrated ability in patient assessment, intake procedures, vital signs monitoring, and case management strategies. Prior experience in geriatrics, behavioral health, pediatrics (PICU), or infectious disease care enhances suitability for this role. Excellent communication skills combined with a compassionate approach to patient service and family support.
