Position Description The Community EMT is a key member of Mosaic's clinical team, providing non-emergency, patient-centered primary care in homes and community settings under medical direction. Working closely with the clinical and wellness teams, this position expands access to care for individuals who are unable to come to our clinic, focusing on prevention, chronic disease management, health education, care coordination, connection to community resources, and reducing unnecessary emergency department utilization. The position offers an exciting opportunity to make a meaningful impact by bridging the gap between emergency services and ongoing community health, ensuring patients receive timely, compassionate, and comprehensive care. Your expertise in critical care, patient assessment, and emergency medicine will drive positive health outcomes and enhance community well-being. Duties Conduct thorough patient assessments in coordination with remote provider within scope of practice including vital signs, physical examinations, chronic disease monitoring and medical history reviews to determine immediate needs and develop appropriate care plans Provide health education related to chronic conditions, medication use, disease prevention, and self-management Working in collaboration with our wellness team, connect patients to community resources addressing social determinants of health (housing, food, utilities, behavioral health, etc.) Serve as a trusted point of contact for patients between clinic visits Focus on preventive care and early intervention to reduce avoidable 911 calls and ED visits Support high-risk patients with frequent EMS utilization, chronic illness, or limited access to care Participate in community outreach and education initiatives Collect and document data related to patient outcomes, utilization, and quality metrics Document all patient encounters accurately and timely in the electronic medical records (EMR) system Maintain compliance with state EMS regulations, medical protocols, and Mosaic policies Participate in quality improvement activities and program evaluation Maintain licensure, certifications, and required continuing education Participate in interdisciplinary team meetings, case reviews, and care conferences Support post-discharge follow-ups to reduce hospital readmissions Collaborate with Mosaic providers and the wellness team to develop and implement individualized care plans Triage patients efficiently to prioritize urgent cases and coordinate appropriate follow-up or transport to healthcare facilities Educate patients on health management, medication usage, and preventive care strategies to promote long-term wellness Operate DME (Durable Medical Equipment) safely during patient visits and ensure proper documentation of all procedures performed Experience Current state licensure as an EMT in good standing with certification in Basic Life Support (BLS) Completion of an accredited community paramedic training program (or willingness to complete within a specified timeframe) Strong background in patient assessment, physiology, anatomy knowledge, and medical terminology; ideally with experience as a community paramedic Minimum of 2-3 years of experience as a practicing paramedic Valid driver's license with acceptable driving record Strong assessment, communication, and patient education skills Ability to work independently in home and community settings Cultural humility and compassion, with comfort working with diverse population Strong clinical judgment and critical thinking Collaborative and team oriented Highly organized and adaptable Familiarity with chronic disease management, behavioral health, and social determinants of health. Experience working with vulnerable or underserved populations. Knowledge of EMR systems and mobile documentation tools.
