- Manage a caseload of hospice patients in homes, assisted living communities, and skilled nursing facilities
- Coordinate care with physicians and the interdisciplinary team
- Deliver expert symptom management and clinical oversight
- Educate patients and families throughout the hospice journey
- Ensure accurate and timely documentation Qualifications
- Active Georgia RN license
- Valid driver's license and reliable transportation
- Minimum one year of nursing experience
- Hospice or home health experience preferred Position also referred to as: Hospice Nurse Case Manager, Hospice Case Manager, Registered Nurse (RN) Care Manager, RNCM, CHPN, Certified Hospice and Palliative Nurse, RN Care Manager, Hospice RN Ready to Make an Impact?
