Job Description

Home Health Care RN Patient is wheelchair bound with G-Tube. Job Overview Join our dynamic healthcare team as a Home Healthcare Nurse (RN) working 8-hour shifts on Saturdays! This energizing role offers you the opportunity to deliver compassionate, high-quality nursing care directly in patients' homes. Your expertise will help ensure safe, effective, and personalized care tailored to each patient's unique needs. Responsibilities Provide comprehensive nursing assessments and develop individualized care plans for patients in their home environment. Administer medications, including injections and IV infusions, following strict aseptic techniques and safety protocols. Perform wound care, suturing, ostomy management, and other advanced clinical procedures with precision and compassion. Monitor vital signs, conduct physical examinations, and evaluate patient responses to treatments. Coordinate with physicians and other healthcare professionals to ensure continuity of care, including discharge planning and case management. Educate patients and their families on health management strategies, medication administration, nutrition, and safety measures. Maintain accurate medical documentation within electronic health record (EHR) systems such as Epic or Cerner while adhering to HIPAA regulations. Experience Valid Registered Nurse (RN) license with current state certification. Critical care experience in settings such as ICU or emergency medicine preferred; hospital or medical-surgical experience highly valued. Demonstrated proficiency with EMR/EHR systems like Epic or Cerner for documentation and patient tracking. Experience working with diverse populations including seniors, individuals with developmental disabilities, memory care needs, or those requiring hospice & palliative medicine. Familiarity with complex procedures such as catheterization, dialysis support, trauma medicine, infectious disease care, and acute pain management is advantageous. Prior home care or community health experience is preferred; candidates with long-term care or assisted living background are encouraged to apply. Knowledge of medical coding (ICD-10/ICD-9), CPT coding, laboratory procedures, specimen collection/processing, and infection control practices will enhance your effectiveness in this role.
